The Forms Section is a complete listing of our current documents that provide helpful information for NHI members and forms to be completed by participants/volunteers.

Included on the forms page are brochures and other guides that are useful to our members. We provide guides for parents, community members, educators, and students.

Also included are release forms that students must return as part of their registration for an NHI program. Should a student wish to apply by filling out a printed application, he/she can obtain a copy of the file needed to complete his/her registration.

Other forms include helpful guides and sample letters that can be used by students who are fundraising their NHI tuition through sponsors. Also included are NHI research materials, travel guides for students attending NHI programs, etc.
